How to Request a Capital Project: Project Request Form Line





Should you currently have a functional need or a program space requirement in your department and you think you may need to complete a capital project (such as a remodel or add some new space) in order to better meet your objectives, you can use the Project Request Form to initiate and approve your proposed project. This includes projects which may be funded by the campus, donors, grants and auxiliary organizations like the Fullerton Foundation and the Philanthropic Foundation. This form can also be used by campus departments to request a potential state funded minor or major capital outlay project.

Please Note: If your proposed “project” entails basic refurbishment like painting, and carpeting or involves repairs or maintenance of your space, please contact Physical Plant to create a work order. This type of work is typically completed by their skilled trades people and does not require a Project Request Form.

The Project Request Form specifies contact information, describes your proposed project, identifies and confirms the source of funds, authorizes AVP Finance - Budget to transfer funds and establish a project account, and confirms appropriate approvals to start the project.

How to Complete the Form:

Project Requestor needs to complete Sections I through IV, and obtain the required signatures in Section V for approval and funding of the project. This form is on “Informed Filler” so you can enter your project information directly into the form and print it for circulation. When completed, please forward the Project Request Form to the Office of Design & Construction:

Section I - Complete Name, Department, and Contact Information:

Please include all information as well as a “Contact Person” should you want to coordinate your proposed project through another individual in your office.

Section II - Describe the project scope, need, funding sources and budget, location and estimated date required to complete project:

You may want to use a written description of your proposed project or add a drawing of your proposed project. Please note that campus building plans can be found on this website under “Campus Map and Building Plans”. You can print these plans if you want to use as a basis of your proposed modifications. When you describe your project include as much program information as possible such as “remodel existing lab in MH-432 into 4 faculty offices 110 sq. ft. each and an administrative support area of 200 sq. ft.” For more complicated projects, it may be useful to do a “Feasibility Study” of your proposed project in order to gain a better understanding of the design and construction issues. Design & Construction can help with a feasibility study if you so desire. (If necessary, a nominal fee may be required for professional consulting)

Section III - Will the proposed project change the current use of space? If yes, describe:

If your proposed project would entail changing an existing space such as a classroom or lab into another type of use such as an office then you need to describe the change of existing space. If you propose to remodel an existing classroom which would remain a classroom then no change use would occur.

Section IV - Indicate the source of funds (account number) and amount to transfer funds for project:

Please indicate your account and amount to transfer. If your department does not have funding, and you would like your proposed project to be considered for future minor capital project funding; then please mark the box provided. Please understand that not all proposed minor capital projects receive funding. Minor Capital Projects are based upon need, type and approval of your Division Head, you many want to first discuss your proposed project with your Dean, Director or Division Head designee before you apply your project for minor capital funding.

Section V - Obtain project signatures and funding approval:

Please note that your Project Request Form needs all (3) required signatures in order to be considered. Incomplete submittals will be returned to you. At least one signature on the form must have signing authority over the account be designated for funding on your proposed project.

Section VI - Office of Design & Construction

will review the proposed project with the Project Requestor regarding budget, schedule and indicate estimated funding to begin and complete the project. If your proposed project is approved by D&C, the Project Request Form will be forwarded to AVP Finance - Budget in order to establish a project account and budget. Non-approved projects will be returned to the Project Requestor. Please be aware that a project request may not receive approval because of conflict with another approved project or if your proposed project does not meet health and safety standards of the university environment or it conflicts with current building codes. Changes in use of space also need to be reviewed and approved.

Section VII – AVP Finance - Budget

will confirm funding source, establish a project account with approved budget, and return the Project Request Form to Design & Construction.

The Director of Design & Construction and Associate Vice President of Facilities Management will serve as the “responsible persons” & “authorized signatures” on the established project account. All funds remaining in the project account will be returned to the project requestor when the project is completed.

Please note that work on a campus funded project cannot begin until the Project Request Form has been completed and approved by all parties, and a project account is established by AVP Finance - Budget. This task usually takes 1-2 weeks to complete.

Design & Construction will notify you when the Project Request is returned and the account is set-up. Design & Construction will assign a Project Manager to your project and will initiate design and construction contracts in order to begin and complete work on your project.
